Has anyone got any advice on how to fill this in as my social worker has sent me a copy to fill in and I'm feeling a bit overwhelmed

    You shouldn’t be filling it in independently ! The social worker should be with you creating it with you in the room. I would contact your social worker and if he refuses request another case worker. Alternatively you can go to citizens advice and they can sign post you to an organisation that can help.
