how did you register it with local hospitals and did you send a copy to your gp thanks
I had a passport from my mental health team, but found it of little to no use. I had to go into hospital several times for a physical condition. First the hospital didn't recognise its validity as it was from a different Trust, as the mental health trust covers a different but overlapping area. So I filled in the hospitals form and it was still ignored by the staff, along with the Hidden Disabilities lanyard.
I ordered one a few years back from the NAS after reading about them. It save time having to explain my medical history. At the time, I had a support worker who completed it for me as my handwriting is so poor I could be a doctor!
I recently did some online autism support sessions.
A Health Passport was made available to us to download and fill in. I thought what a great idea.
Anything that will help in a stressful situation has got to be worthwhile.

I have a health passport, I was supported to make it by my local autism services at the time
It has been useful for outpatient procedures thus far, and it is registered in my local hospitals. I've yet to try it out of area
