hi wondered has anyone got a health passport that you show to any clinical staff, if so has it helped and did you get help to fill it in

    My local hospitals have one that they use that my autism services helped me fill in and then it got sent by email to the learning disability and autism team at the hospital trust who filed it, and updated it the one time it needed updating.

    The learning disability and autism team also spent a specialist nurse to support my care provider to support me the one time I had a really distressing procedure
