It could be what's behind it eludes you, and he just doesn't understand it's self-harm yet. My mom once found me 'cooking my hand' when I was 5, while I was just checking if the water is boiling, that was what she sent me to do, and I wasn't sure how to decide if its hot enough or no.
Or he does is with full intent because something around irritates him. Some noises can be silenced by banging your head.
