hello and help

I am 52 years old and was only diagnosed with Aspergers 3 years ago, the diagnosis has helped me understand many of the problems I have faced in life.

I have been given a 'health passport' from my local NHS Trust and would appeciate any advice in filling it is. I am supposed to say what help I need when attending hospital or being admitted.

I do not function well in groups, or when  I dont know th esurroundings, hospitals are something of a phobia, I cnnot cope in h]busy witing rooms or wards where tere are kot of people.

If any body is willing to buddy me to fill in this passport I would be very grateful

    I do not have a template as such because I made it up myself as I went allong. But I could try to give instructions with what I did, that you could perhapse change slightly to fit in with yourself.

    I started writing it when I was in my last year at school. So I was in a place I knew well, with people who knew me and who I knew. But, as i was asked the idea of it was something to give to new teachers so they know about me and how to help. So I sat in lessons at school and imagined myself in the same lesson and class that I knew but with a completly new teacher. Then I asked "what would I want this teacher to know about me that would help me?" or it could be changed to "what does my normal teacher do to help me?" Or other similar questions. I also asked "What could cause me a problem if I was with people I knew but in a different place?" So I broke down the whole situation and focused on one thing each week. I wrote all these answes down and then turned it into a list of information about me. I did the same for all the other times in school such as moving between lessons, tutorials, assemblies, lunchtimes ect. 

    So, could you do something similar? I imagine you have no chance now to be where you would be but could you imagine you are in hospital and answer similar questions. such as "what would I like the nurses to know about me to help me?" or think to a nurse you really liked or disliked. what did they do that was good or bad. Include things like that. It can be anything from what you like, don't like, what you can or can not do or what you need help with. Or anything else you think it would help if people knew.

    To the seccond thing about asking for a buddy. I heard you can get a buddy through the NAS but I do not know exactly how. They have people that do it in specific areas and also an online buddy is something I have heard about. I just tried searching the website and can not find it but someone else may know more about it. Or you could use the advice service to ask them about it.
