My situation has been a negative one. I disclosed and now almost 3 years later I am waiting on a date for an employment tribunal. My employers looked on my requests as perks and had no intention of helping. I think the advice of checking out the lay of the land first is good. If you have an occupational health department, make an appointment and disclose to them, it provides legal protection.
There are many positive and negative examples of disclosing. I was lucky / unlucky that I am in my fifties so I have less of a career to worry about. If you are young then you may want to figure out what exactly your issues are at work and what will really make a difference before embarking on the disclosure voyage.
