Does anyone know where I can get an invisible disability card from for Autism and adhd as I am looking to get one and not sure where I can find them

By the way, I’ve vaguely heard about a travel card that one can get for either free or discounted transport costs (I’m in Manchester) but I don’t know if there is one for the whole of the U.K. as well
