Even if information you gain through the questionnaire is kept anonymous, information within could indirectly identify a person or a service. Therefore you have to be able to assure people the evidence wont be left lying around or shared with third parties. This is especially crucial if the data concerns minors.
If you are doing this as part of an undergraduate course, as a dissertation or project, it should be checked by your supervisor for ethical implications. Unfortunately a lot of universities seem to be allowing this to happen now, whereas in the past it was taboo.
Forums like this one are safe places for people to confide their problems with others. They shouldn't be used as a quarry for student projects.
