Desperately looking for advice filling in a referral form

Hi, 

I have finally got an autism referral form for my 7 year old from my his school after trying to start this process for the last 2.5 years. But now I have it, I don’t have a clue where to start. I’m struggling to fill in the form. I’m scared of letting my child down if I don’t do this properly. I have looked up places to support me with this, and I have emailed a bunch of them and all that happens is I get sent a list of different places to contact… who then do the same. 

My other child (6) is already on the waiting list for an autism assessment, but this was completed entirely by a paediatrician as he was referred there first due to other issues, and being more ‘obviously’ likely to have autism. Whereas my 7 year old masks at school and does not have other obvious conditions.

I am not diagnosed or even on a waiting list, but I strongly suspect I am autistic too which may be contributing to the difficulty. I don’t feel I can use support services related to this though as it’s only my opinion realistically.

Does anyone here know where I could possibly get help to fill in an autism referral form?

  • Hi Jax94. That sounds incredibly frustrating being passed from one place to the next. I would suggest the Social Prescriber/Care Co-ordinator at your GP surgery, but have you already been bounced from them?

    Is it an AQ10 form that you have been asked to do? Or more in depth?
