I get DLA for my son who is 3, we get middle rate as he also has significant development delay.
Appeal the decision!!!
I also used a local carers centre to help with the forms (although I work in a government department and feel comfortale doing it myself I was advised to use them by professionals working with my son).
I was initially refused (without them contacting any of the professionals involved!) and I rang them to advise that I would be appealing - when they asked why I told them. I then prepared my paperwork and rang them 4 days letter to request a fax number or e-mail address for me to send it to and they said based on the phone call alone they had revised the decision and awarded middle rate.
From what I have heard from others this seems to be a very common thing that happens.
