This is unprofessional of NAS

mods do you agree?

I completed a risk assessment with a NAS outreach manager to go on a trip with a support worker, there was a part I disagreed with so it was agreed to have been left out and the document was printed out and I signed it.

Recently I visited the office to help update my support plan documents and saw the risk assessment had been changed after I had signed it without notifying me, a section was added in which suggested there was a risk of something which I had disagreed with. I had already signed the document so why was it changed after I signed it without anyone telling me?

I am not happy.
