The forms I've been told are also known as borders forms, or ' care home ' forms

Rhys goes to full time school, home weekends and holidays.

The official website has the recognised forms www.gov.uk/.../disability-living-allowance-for-children-claim-form

They all have a prefix DLA. A form DBA sounds suspect. All their forms should be on the web.

DBD doesn't seem to come up at all, so its not even the wrong form for the context.

The fact that the notes don't accompany the form adds to my suspicion this is a bogus form. You should complain to DLA, and if possible copy to your MP or local councillor.

All such procedures should be accessible and transparent. But then who knows, with the attitudes to disability being demonstrated by the current Government, who knows what they've dreamt up......

But all the same, whatever new machinations, you should be able to find out about the form on-line, if you cannot, something is wrong.
