Don't know where to start

I am looking for some help for my family, we seem to be going backwards, and I'm worried about it. I'm starting to research this area, but don't really know where to begin.
 
Some background to the situation: My brother has always had certain quirks about him, but these have become exaggerated, particularly over the last 5 years. He can't hold a job down and has started living in his room, hoarding collections of keepsakes around his room, which he doesn't like to leave. Tantrums are becoming more regular, with shouting and sometimes violent behaviour.
 
My parents are trying to help, but as they do not know much about the condition his condition seems to be getting worse. They have tried to get him to a doctor for the last 3-ish years, but he went, and refused to tell the doctor anything, so the doctor sent him away, and nothing more has been done.
 
Essentially, this situation is tearing my family apart, and so I am trying to do something about it, firstly I am trying to put some information together about diagnosis, and am trying to find out if a home diagnosis is possible, as getting him out the house often results in tantrums. Also it would be great to find out if there is any support for my parents, to help them learn how to deal with the circumstances of the current situation.
 
I'm trying to make life a little easier for them all.

    Im sorry your all going through this, you could also try ringing your local social services, and ask for the the adult services team, they have different names in different areas,  and eventually you should get the right person, perhaps they have a community support team who will be able to come and see your brother at home.
