Blue badge- hidden disability questionnaire

Hello,

i have applied for a blue badge due to my 5yr old not being danger aware and often running from places. We often drive to most outings as public transport is overwhelming. I arrive at the school very early to find a suitable parking space and sometimes


I received a form for hidden disabilities which they would like back in 3 weeks however he has just broken up from school and I have received this in the post today. 

he is under a consultant for bowel/bladder and has had an ASD assessment, but I have not had an official diagnosis as of yet.

Can the GP help?

  • If there are no instructions or guidelines for completion of the form, you could contact whoever sent you the form to ask for guidance about completion and deadlines. (Might the school office be open for at least part of the summer holidays?)

    If they are looking for proof of your son’s hidden disabilities, and your GP has been involved in this aspect of your son’s care, you could contact them.
