Most medical professionals don't intend to cause harm. Procedures may be recommended, but not work on all occasions. It does not make it intentional. It may that some staff are a bit cold or uncaring, but this could be for a number of reasons, and is not necessarily malicious.

For mental health where you can only go on external symptoms, it can be difficult to tell what certain medication is doing if the patient does not say.

Friends and relatives also may have good intentions but not knowing what is happening. If someone appears more calm and less stressed, is this good or bad? They can't know what thoughts or feelings  go on underneath.

Other people may look, or make comment, or be mean and hurtful. It is either because they are thoughtless, some might think it fun because they are not very nice, but for some it is because they are scared it might happen to them and they don't know what to do.

So people have not generally tried to make things hard, even if it may look like it.
