Social services through your local council is probably your key contact point. You could also Google to see if there are any charities in your area that can provide support.
If your son is unable to live independently, then my advice would be to contact your local council and ask what help is available.

This kind of thing can be very area dependent. It also depends on how independent he can be. There are places that offer supported living either at a cost or through social services. This is better for those that would struggle to live independently at all. There also options for those that would be able to do a lot independently but not completely through the means of things like PAs again either at cost or through social services.
