Advice for DSA

Hello lovely people - can you help? I have looked at getting the DSA (Disabled Students' Allowance)  to help support me next year at university. However, I have no idea what evidence needs to be submitted, as different websites all seem to say conflicting things. Do I need to send a diagnostic report, a doctor's letter, the disability evidence form or something else? Any advice would be much appreciated :D

    So pleased it's not just me struggling with this. I found applying for a student loan really easy however DSA is really complicated. I was also concerned about the proof I had to send. I have taken the advice below and attached my assessment as proof, I previously phoned the doctor and asked about a letter and apparently that would cost £42. I'm hoping that the assessment is good enough.
