What accommodations can I ask for in an interview?

I struggle processing verbal and written communication in situations where I am anxious, particularly interviews. I have already informed them on my application that I have a disability but they didn’t ask if they could make any adjustments which has concerned me a little. What reasonable adjustments and accommodations have others asked for in previous interviews?

I have been told I will be asked to complete an exercise before the formal interview so I would like more information on what this exercise is. 


I have started questions to email to the interviewer, see below.

What is involved in the exercise I will be required to complete on arrival? 
What is the format of the interview?
Will there be a fixed list of questions for all candidates?
If there are fixed questions I would appreciate if these to be provided before the interview to allow me time to process them. 
Will answers be pointed or scored?
During the interview I may need time to respond to answers and will be able to respond with more detail if asked about real experience rather than abstract situations. I may also answer multilayered questions in parts, ask for questions to be repeated  and will benefit from writing down questions.
Has anyone else found any other adjustments that have been helpful?
