assessment questionares

hi im just filling in my assessment questionares, some are fairy easy the ones where you just circle around your answer, but the other 2 forms are sending my brain into a shutdown.  the box to answer in is tiny so i cant write what i need to and some of they questions i just dont understand.  one says i avoid certain tastes or smells please list which i understand but then next one says strong taste or smell preferences may be non ediable please list .   are these asking the same thing im confused.

  • You could, and indeed I did, continue on another sheet of paper with as much detail as possible.

    If you aren't sure what the question means, I'd say so but then write what you think you understand it to be. They seem to be inviting you to list your sensory responses, both the things you avoid because they are unbearable and the things you seek out. The sensory profile can swing both ways; hyper sensitive or hyposensitive. That is feeling too much or too little of something.

  • Hi, I think they are asking about non food items that we like to chew on. I would often chew paper at school, some girls liked to put their hair in their mouths and chew.  I loved the smell of new plasticine, Chewing the end of a pencil all are stimming items that you put in your mouth or smell. There is a name for it which I can’t remember. I did my forms and added pages from Microsoft Word, much easier to explain things and then just attached them. Some of the questions can be a bit repetitive. My doctor ended up with four A4 pages of my answers. It’s worth taking time over as if your refused a referral it then becomes very hard work. Good luck.
