Can someone please.proofread this to make sure I have written it right?

I am applying for a DSA for university but I don't have a diagnosis, however I am on the waitlist. I have been told that if I have a DSA evidence form filled out as a "working diagnosis" then that should be fine. I was hoping that the doctors had started in person consultations again but they are all still just telephone consults. I have gon onto the doctors medical practise where I go and they do inline consults now. I have filled out something that tells them what I need doing. Would someone mind proof reading it and making sure I have written it right? I would show my mum but she's at work and I do t think my dad even knows I am on the waitlist for an autism diagnosis because when we were going through the student loan calculator and it asks you if any of these things apply, one of them was if I had a disability. The other 2 options were if you had a child and things like that. He told me to click on none of the above. I wanted to tell him but the words got stuck in my chest (like at school when you know the answer but you don't want to say it in case you are wrong, even though you know for sure you are right).

This is what I have written (it can only have 300 characters) (I tried to put in a screenshot but it isn't working properly):

    Have you already had your assessment?

    If you haven’t, I would say:

    I am going to university, and need a DSA evidence form filled out. I am on a waiting list for an Autism assessment. I suffer with panic attacks, and after a test at college, they discovered I have dislexic tendencies.

    You spelt dyslexic as dislexic. I would include the incorrect spelling, as it shows the problem adequately.

  • Your not indruing at all. I haven't had an assessment yet. The waitlist is so long and it is quite frustrating. All I can do is wait for it because my family doesn't have the funds to have it done privately.
