Got my assessment referral letter today. Now I can relax a bit!

I spoke to someone at my GP surgery a few weeks ago, who told me the NHS Trust I've been referred to is being restructured so there have been some delays in contacting people about their assessments.

But I got a letter today saying they've received my referral and I'll be getting an appointment with them once confirmation of funding comes through.

I've been a bit on edge while I've been waiting to hear from them, so that's one less thing to worry about.

I've got loads of notes ready, and some old school reports.  Is there anything else that it might be useful to have?  (It's not going to be possible to ask family members to attend with me.)
