Pre-diagnostic assessment form?

Did any one else have difficulty completing their pre-diagnostic assessment form?

On the form they only gave me very small spaces to answer the following questions:

What are the main difficulties that you are experiencing?

How would an Autism diagnosis be helpful?

Please use this space to provide any information (eg about your difficulties/reason for wanting a diagnosis, life) that you think is important for a clinician to know.

I have so much I need to say. They didn't mention anything about using extra sheets if necessary, but I have done. I can't stop myself, and I don't want to risk not mentioning something that may be important, and that I may forget to mention during the actual assessment interview. I generally communicate so much better in writing, compared to how I communicate verbally in person.

I'm trying not to go so ridiculously over the top that they will be put off. I keep on adding, editing, and improving the document as I remember things or discover new things.. The document is 8 sheets so far and I'm trying to keep it as brief as possible, and trying to be strict with only mentioning things that I feel are strictly relevant.

The assessors really may well not like this, as this is not what they asked for, but I need to do it, I need to document everything and get it all on paper in a coherent and cohesive format. It's helping me make sense of the whole thing, and it's good to bring together all that I have learnt and how it relates to me, in a format that gets all of the facts, and all of my thoughts, organised and together.

They have said that I am on the waiting list. In the meantime,  I would like to just get this document completed and literally draw a line under it and get it sent off. Lest I never get it completed for fear of missing something out, or for fear that I've not put my points in the right places and in the right order. Trying to get it as perfect as possible. I've already had the form for 2 months, but I have been busy with other things, and am just working on the document as and when I get chance.

  • I scribbled all over the form, included lots of annotations.

Also, I’ve typed over 50 pages about my life, edited to 36 pages and sent them before the assessment. About everything because they should know what is relevant and what is not and it’s better to include something irrelevant that omit something important.
